For an IPv6 related paper we are currently working on, Claudio and I are doing a small online survey on the use of IPv6 among OpenBSD developers and users.
It would be nice if you could spare 10-15 minutes of your time and answer the questions. Please do that also if you don't use IPv6, since that helps us evaluating how much it is used.

I second this, for two reasons...
1) I would love to know what has prevented others
2) I would love to explain all the details of what prevented me. I realize this is info that couldn't easily be tabulated, but it might have provided more insight overall.
None the less, a good survey. I look forward to reading the results.

The biggest reason for not having IPv6 at work (a hosting provider), there aren't enough transit-providers we can connect to to have a redundant IPv6-connection.

Maybe where you are located, but there is no lack of transit providers in general.

We are in 2 locations and at one there are 0 transit-providers that have IPv6.
Pretty sad really.

I think it's unlikely core stuff like DNS, MX, etc will be able to go v6-only any time in the forseeable future. Everyone has to support v4 on both sides, so it's tough to justify using v6 here.
I'm skeptical publicly reachable servers will be exclusively v6 in any significant way in the next decade.
